Form Name Description
IRA Application Open a new Traditional or Roth IRA.
IRA Transfer of Assets Form Use this form with your IRA Application when transferring a Traditional or Roth IRA from another institution. Also, use this form with your IRA Application to rollover all or part of your retirement plan from your former employer if you are retiring or changing jobs.
Education Savings Account Application (formerly Education IRA) Set up an Education Savings Account to save for a child's college education.
Education Savings Account Transfer of Assets Form Use this form with your Education Savings Account (ESA) Application when transferring an ESA from another institution.
Roth Conversion Form Use this form to convert your existing Oakmark Traditional IRA assets to an Oakmark Roth IRA.
IRA Distribution Form Use this form to request an IRA Distribution.
Shareholder Services Form Use this form to add or change the following:
    * Address
    * Social Security or Tax ID Number
    * Electronic Transactions
    * Bank Information
    * Distribution Options
    * Systematic Withdrawal Plan
    * Automatic Investment Plan
Change of Beneficiary Form Designate or change your IRA beneficiary.


Investment Minimums

Initial IRA investment minimums
  • $1,000 per Fund account
  • $500 if an Automatic Investment or Payroll Deduction Plan is established
  • $100 subsequent purchases
Initial Coverdell Education Savings Account minimums
  • $500 initial investment (per fund account)
  • Under the current rules, you can invest $2,000 per year per child (under age 18) to pay for certain education expenses. This includes tuition, books, supplies, and room and board for students enrolled more than half time. The money you invest in Coverdell Education Savings Accounts does not count against the maximum you may invest in your other IRAs. Eligibility to contribute to a Coverdell Education Savings Account follows the same income guidelines as the Roth IRA.
